Your  name/penname: what I and others will address you in ooc conversations 

Your e-mail:  speaks for itself.

Name: Your character's name. (i.e. Joe Smith, Anna Black)

Code Name:  C'mon, you know you love these. As corny as they sometimes can be, your mutant's codename. (i.e. Quicksilver, Toad, Avalanche, The Blob, Boom Boom...)

Nick Name (optional): other than the codename, what is your character's nickname(s)? (i.e. Alexandra can be Alex or Zee.) 

Gender: 

Age: 

Appearance:  appearances are everything! Are they tall, short, brunette, blonde.. etc.

Personality:  This is crucial. How would they respond to different situations? be specific

Affliction: X-Men, Brotherhood, or none?

Hobbies: What do they do in their spare time?

Powers:  (please keep them to  one or two powers only. )

Family:  What type of family life do they have? Do they have any siblings? Are they related to the x-men? (of they are, please notify me, as it may be changed. We don't want any weird inbreeding going on here. ^_^)

Other:

Quote: If evo. character from the show, quote from there. If original character, make up a quote that they are likely to say.
